Formed by Qualidigm in 2012, the Connecticut Partners for Health work together to align healthcare quality improvement and patient safety initiatives in Connecticut.
In Connecticut, providers across the health care continuum were frequently asked to participate in healthcare quality initiatives from varying stakeholders. As a result, many healthcare initiatives were duplicative.
In an effort to align, rather than duplicate healthcare quality initiatives, Qualidigm formed the Connecticut Partners for Health in 2012. Consisting of over 40 Connecticut healthcare stakeholders representing healthcare providers, trade associations, consumer organizations, health plans and payers, community-based organizations, academic institutions, government agencies, voluntary health organizations, the Regional Extension Center, the business community, their mission is to engage consumers to become active, informed partners in managing their health and healthcare.
With a focus on patient engagement the CT Partners for Health hosts large scale educational forums for both providers and healthcare consumers related to healthcare quality improvement initiatives. The group also acts as a portal to disseminate individual member’s events and activities.
