Lead a collaborative of several hospitals and their community partners to address the threat of antibiotic resistance, and implement antimicrobial stewardship programs across the continuum of care.

Challenge:

A growing number of healthcare acquired infections (HAIs), including MRSA and c. difficile, are caused by antibiotic resistance, which is directly linked to widespread overuse and incorrect prescribing practices. Antibiotic stewardship programs help to ensure that patients get the right antibiotics at the right time for the right duration. Numerous studies have shown that implementing an antibiotic stewardship program can not only save lives, but can save significant health care dollars.

Solution:

The State of CT Department of Public Health (DPH) selected Qualidigm to lead a collaborative of several hospitals and their community partners (Communities of Care) to address the public health threat of antibiotic resistance. Qualidigm guided participants in implementing antimicrobial stewardship programs across the continuum of care by:

  • identifying community-specific issues
  • sharing evidence-based practices
  • providing technical assistance
  • leveraging peer-to-peer mentoring
  • tracking multi-drug resistant organism (MDRO) rates
  • developing resources
  • conducting large group educational forums

The collaborative measured and promoted the appropriate use of antimicrobials by selecting an appropriate agent, dose, and duration to improve patient outcomes, minimize toxicity, and prevent emergence of antimicrobial resistance.

Benefit:

Each hospital and their community partners implemented an antimicrobial intervention across the healthcare continuum, breaking down health care silos and increasing communication between settings. Key issues around antimicrobial use that were statewide were identified and peer to peer sharing was facilitated.
