Patient Safety

For more than a decade, the company has been working in the area of patient safety at both the state and national levels in formal programs to identify opportunities to improve on the safe delivery of care as well as to develop and implement solutions to minimize the potential of harm to patients.  It is also working with providers, either in groups or as individual entities, to achieve the same outcome.

On the national level, Qualidigm, in concert with the Centers for Medicare & Medicaid Services (CMS) and now with the Agency for Healthcare Research and Quality (AHRQ), has designed and implemented the only national Medicare surveillance system to identify and monitor patient safety issues in hospitals.   The database has been operational for the past eight years and continuously contributes to patient safety policy-making and programs to reduce the risk of patient harm.  Read more.   

At the state level, Qualidigm is designated as a Patient Safety Organization (PSO) by the Connecticut Department of Public Health to work with hospitals and selected ambulatory facilities to reduce the risk of patient harm.  Read more.  In addition, Qualidigm is funded by CMS under its Quality Improvement Organization (QIO) contract, to work with its community partners to reduce patient harm in selected areas, such as reducing the potential for facility acquired infections in hospitals and nursing homes.     

The patient safety services Qualidigm offers are available to any client: providers, government agencies, researchers, foundations, health plans, and business. Qualidigm has a staff of knowledgeable, skilled and experienced professionals who work together, integrating their talents based on the need of each client, to offer services that may include but not be limited to:

  • Analysis of data to identify and quantify issues and concerns
  • Root cause analysis to identify the challenges/areas requiring change
  • Design of action plans based on patient centered, evidence-based medicine and interventions
  • Providing support through the plan implementation process
  • Data collection and analysis of action plan outcomes
