Electronic health records (EHRs) give physicians a powerful tool for preventing errors, measuring performance, and sharing health information with patients and other providers. As more clinicians adopt EHRs and make them part of the patient care process, they also are relying on them to coordinate individual care and manage population health.
As the Quality Improvement Organization (QIO) for Connecticut, Qualidigm assists primary care physician practices with clinical data reporting to leverage EHR functionality to increase rates of preventive services, decrease cardiac risk factors, and prepare for value-based payment by Medicare and other health insurers.
