Payers are beginning to recognize practices that achieve this designation. This recognition is usually in the form of financial incentive. With this in mind, and the success of the initial effort, Qualidigm will be offering this service to additional practices in the future.

Qualidigm has initiated a service to assist physician practices to achieve designation as Patient Centered Medical Homes (PCMH). PCMH is a program designed to improve primary care by organizing care around patients, working in teams of individuals who are providing the care, and coordinating and tracking their care and progress over time. It facilitates a partnership between a patient and their primary care physician, and the patient’s family, when appropriate.
Accrediting bodies, such as the National Committee for Quality Assurance (NCQA), have developed specific standards that practices must meet in order to be designated as a PCMH. To meet these standards, practices are required to file an application with the accrediting body. The application is extensive in defining the services that a practice must have in place and requires actual documentation of the delivery of these services as part of the application.
Qualidigm, in collaboration with the Connecticut State Medical Society (CSMS) and the Connecticut State Medical Society-IPA (CSMS-IPA), has already assisted a group of practices to achieve PCMH designation through the NCQA program. This initial effort was funded through a grant from the Physicians Foundation to the CSMS.
In an effort to continue the adoption of PCMH by practices throughout the State, the CSMS-IPA is sponsoring an additional group of practices to prepare for recognition. Once again, Qualidigm will provide both webinar based and direct assistance services to these practices with the goal of NCQA Recognition.
