Director, Consulting Services, Long Term Care
Immediate full time opening for individual to direct day-to-day operations of assigned healthcare quality improvement projects in Qualidigm’s Long Term Care area of practice,. Responsibilities include directing, developing, implementing, and administering operations strategies and objectives to ensure the achievement of contract deliverables, as well as the hiring, management, development and guidance of assigned staff. Serve as liaison with external collaborators on behalf of Qualidigm. Participate with the project teams to design workshops, interventions, and preparatory materials. Provide quality improvement expertise in the development, implementation, and evaluation of workshops and interventions. Advise customers on implementation of effective evidence-based quality improvement interventions and practices. Teach and coach practices in process improvement at workshops, via face-to-face, teleconferences and webcasts.
You will need a bachelor degree in a healthcare related field; graduate degree preferred. You must have comprehensive knowledge of long term care, a strong background in health care quality improvement processes, and at least five years’ direct management of large-scale, complex projects and contracts in a healthcare environment, including staff supervision. Experience as a trainer, including responsibility for developing educational programs for adult learners is also required. At least one professional certification is desired, such as Project Management, healthcare quality, nursing, nursing home administration, etc.

Nurse Case Manager
Immediate full time opening for Nurse Case Manager for the Connect Care Choice Program in Rhode Island. Provide comprehensive screening, assessment, care coordination services, disease education and self management support to moderate to high risk Medicaid-only adults with complex chronic health conditions, in a community primary care practice setting. Conduct face-to-face interviews in a variety of locations (i.e., physician office, patient homes, other clinical settings) to assess baseline knowledge of conditions; determine patient strengths/skills; identify patient’s support system and current community supports; and patient’s identified needs and barriers. Act as a liaison between practice and members when hospitalized at unaffiliated facilities in order to communicate admission information provided by DHS to PCP to facilitate discharge planning and follow up. Establish care plans, goals, interventions and contact schedule based on risk category, and patient’s and family members’ identified medical and social needs. This position is home-based, with daily in-state travel to physician offices and/or patient homes, requiring a flexible work schedule in order to accommodate patient meetings and appointments outside of normal business hours.
A Bachelors degree in nursing, or the equivalent in combined formal education and experience is required. Three to five years experience in community health setting, public health, chronic disease management, community nursing, case management preferred. Experience must include knowledge of case management, disease and care management and chronic care principles, as well as working with primary care providers in a practice setting to coordinate care and disease management. Candidate must have a valid Rhode Island license as a Registered Nurse and a valid driver’s license. Intermediate computer proficiency in the use of Microsoft Office (Word, Excel, Access, Outlook), web-based applications, and the Internet required. Strong oral & written communication skills also required.
