EQual Health Care Project

Qualidigm is recruiting 12 Greater Hartford private physician practices which serve large minority populations to participate in the Connecticut Health Foundation-funded Equity and Quality in Health Care “EQual Healthcare Project.” Connecticut Health Foundation awarded the $592,000, two-year grant to Qualidigm to improve patient-provider interactions and enhance quality improvement knowledge and systems for private physicians whose practices serve diverse clientele. The grant is the largest one awarded by the Connecticut Health Foundation in 2009.

Qualidigm’s support to the primary care physician offices recruited for the EQual Healthcare Project will include, but not be limited to, needs assessment, outreach visits, new technology implementation such as an electronic patient registry able to produce individual and population health data, patient education tools, cultural competency training, community resources and impact measurement.

“Cementing the provider-physician relationship through more culturally-sensitive and linguistically-appropriate tools is essential to making more equitable, measurable improvements in the health and well-being of individuals, their families and local communities,” said Patricia Baker, president and CEO of the Connecticut Health Foundation. Baker noted that the incidence of Diabetes is one of the most pervasive and costly health care issues among Black and Latino populations.

Connecticut Health Foundation awarded the $592,000 grant to Qualidigm in January 2009 for which physician recruitment is under way. The EQual Healthcare Project‘s principal investigator is Thomas J. VanHoof, M.D., a faculty member at the University of Connecticut School of Nursing, who is working in close collaboration with Thomas P. Meehan, M.D., MPH, FACP, Qualdigm’s chief medical officer. The Equal Healthcare Project will continue through December 2010 and may broaden to include health conditions beyond Diabetes as the project evolves.
