Target Setting and STAR

Be the STAR of your own show!

Qualidigm invites you to join nursing homes across Connecticut and the country in setting performance targets on the STAR website, www.nhqi-star.org, developed by the Centers for Medicare & Medicaid Services (CMS).

Why set targets? As Abraham Lincoln said, "A goal properly set is halfway reached." Targets help you set priorities, identify problem areas and create your own roadmap for improvement. You can use the STAR website's trend data to monitor the effects of new approaches and tools that you implement. STAR is secure and confidential. Only your facility will know what targets you set.

Set your own targets for these important measures:

  • Physical restraints.
  • High-risk pressure ulcers.
  • Depression.
  • Chronic pain.

Check the STAR website quarterly to track your own progress.

  • View your nursing home's quality measure trend report.
  • Track your achievements over time.
  • Compare your performance to state and national peers.

Setting targets on the STAR website is quick and as easy as 1, 2, 3.

  1. Go to www.nhqi-star.org, create an account (or enter your username and password from last year) and log in.
  2. Follow the instructions to select each measure and define a target.
  3. You'll be asked how you want to calculate your targets. Choose from a variety of improvement methods for all four measures.
  • Each method offers different advantages. A simple choice is the "percent improvement" method, using 10% or 20%.
    The software will calculate your specific targets for you.
