Information Collection and Use

Qualidigm is the sole owner of the information collected on this site. Qualidigm does not share, sell or trade user information (e.g., e-mail address, name, address, phone number) to any organizations, groups or entities. If we need to share demographic information with third parties, when contractually required, we will give them aggregate information only. Qualidigm collects and records the following information about each website visitor:

  • Date and time of visit.
  • Where the visitor goes on the Qualidigm sites.
  • Type of computer (Mac, PC, Unix) and browser used by the visitor.
  • Search engine used to find site and keywords which brought visitor to the site.
  • Location of visitor's machine (by country, not street address).

With this information, Qualidigm can track not only the number of “hits” (visits) to the website, but also the times of day, the sections that are the most popular and the range of computers and browsers most commonly used by visitors. Qualidigm does not use any of this information to identify or track individual visitors.

Meeting Registrations

Online registration may be available for some professional meetings or special events. During registration, users are required to give their contact information (name, credentials, organization, address, phone and e-mail address). This information is used to contact and register the user regarding the meeting. It may also be used to contact the user regarding future Qualidigm meetings and activities. Required information fields for registration are clearly marked; all other fields are optional.

Cookies

What is a cookie? A cookie is a small piece of information that is sent to your browser, along with a web page, when you access a website. You do not need to have cookies enabled to visit the Qualidigm website. There are two kinds of cookies. A session cookie is a line of text that is stored temporarily in your computer's memory. A session cookie is never saved but rather is destroyed as soon as you close your browser. A persistent cookie is a more permanent line of text that gets saved by your browser to a file on your hard drive. Qualidigm uses only session cookies. We do not use any persistent cookies.

Sharing

Qualidigm may share information on the use of its sites with some of its customers, such as the Centers for Medicare & Medicaid Services, some state governments and others. Qualidigm does not provide personally identifiable information in these reports.

The Qualidigm website contains many links to other sites. Please be aware that other websites that may be accessed through our site may collect personally identifiable information about you. Qualidigm is not responsible for the privacy practices of other sites. Users are encouraged to be aware when they leave the Qualidigm site and to read the privacy statements of each linked website. The Qualidigm server is kept in a secure, restricted access environment. Policies and procedures are in place to ensure the security of user information. Ongoing training on security and privacy practices is provided to Qualidigm staff and consultants.

E-Mail

While Qualidigm will make every attempt to protect the personal information that you share with us, electronic mail is not secure against interception. If your communication is very sensitive, you may want to send it by United States mail instead.

Qualidigm reserves the right to amend this policy as necessary, without prior notice. We will post Privacy Policy changes on our websites in a timely manner.
